MEDICAL AUTHORIZATION, MEDIA RELEASE, AND VOLUNTEER AGREEMENT In consideration of being permitted to volunteer for, assist with, enter, access, attend, participate in, or otherwise engage in activities related to the Pistons & Props Airshow and/or Pistons & Props Car Show (collectively, the “Event”), I, the undersigned volunteer, intending to be legally bound, do hereby waive, release, and forever discharge any and all rights and claims for damages, losses, demands, injuries, causes of action, or liabilities that I may have against the Event Director, Propel Forward, Inc., Southern Wisconsin Regional Airport, the Southern Wisconsin Regional Airport Board, Rock County, the Airport Manager, event sponsors and their representatives, event partners, volunteers, staff, employees, contractors, committees, agents, designees, successors, and assigns, and any other persons or entities assisting with or associated with the Event (collectively, the “Released Parties”), arising out of or related to my volunteer service, attendance at, entry into, or participation in the Event. This release includes any and all injuries and damages suffered by me before, during, or after the Event, including injuries to my person and damage to my personal property. I understand and intend that this release shall be binding upon my heirs, executors, administrators, legal representatives, successors, and assigns. I acknowledge that I am serving as a volunteer and not as an employee, contractor, or agent of Propel Forward, Inc. or any of the Released Parties. I understand that I am not entitled to wages, salary, employment benefits, unemployment benefits, workers’ compensation benefits (except as otherwise required by law), retirement benefits, insurance benefits, or any other compensation arising from my volunteer service. Any volunteer appreciation items, meals, merchandise, admission benefits, or other incentives provided to volunteers are not compensation and do not create an employment relationship. I understand that volunteering at an airport, airshow, car show, or related event may involve known and unknown risks, hazards, and dangers. I should not volunteer unless I am medically able to do so. I knowingly and voluntarily assume all risks associated with volunteering, attending, or participating in the Event, including but not limited to low-hanging aircraft wings, propeller hazards, aircraft operations, fuel fumes, fire hazards, loud noises, moving vehicles, golf cart operations, parking operations, equipment movement, setup and teardown activities, lifting and carrying materials, crowd-control activities, weather conditions, traffic, airport conditions, contact with attendees, contact with participants, falls, slips, trips, exposure to dust, heat, cold, rain, and all other risks typically associated with airport, airshow, car show, parking, admissions, hospitality, operations, maintenance, and event-support activities. I acknowledge that these risks are known and understood by me, and I voluntarily accept full responsibility for any loss, property damage, personal injury, illness, or death that may result from my volunteer service, attendance, or participation. I understand that volunteer assignments may include, but are not limited to, admissions, ticketing, parking, crowd management, hospitality, VIP services, setup, teardown, equipment handling, grounds support, information services, transportation assistance, merchandise sales, educational activities, registration, administrative support, and other event-related duties as assigned. I agree to perform only those duties authorized by Event management and to follow all instructions provided by Event staff, supervisors, airport personnel, security personnel, law enforcement personnel, and authorized representatives. I agree to comply with all safety requirements, operational procedures, airport regulations, restricted-area requirements, traffic-control procedures, and Event policies. I will wear any required volunteer credentials, identification, safety equipment, or apparel as directed. I agree to immediately report unsafe conditions, injuries, accidents, incidents, or emergencies to Event management. I understand that failure to follow safety procedures may result in removal from volunteer service. I agree to abide by all instructions and decisions of Event staff, airport personnel, security personnel, and authorized representatives. I certify, as a material condition of my being permitted to volunteer, attend, or participate, that I am physically fit and of sound mind and will not create a hazard to myself or others. I understand that volunteer service may be terminated at any time, with or without cause, by Event management, and that Event management may reassign volunteer duties at its sole discretion as operational needs require. I further assume all liability for damages, including but not limited to personal injury, illness, death, and property damage, that may occur to myself, my family members, my guests, or any others arising from my volunteer service, attendance at, or participation in the Event. On behalf of myself and my heirs, executors, administrators, legal representatives, successors, and assigns, I agree to indemnify, defend, hold harmless, and release the Released Parties from and against any and all claims, liabilities, causes of action, costs, expenses, or demands arising from or related to my volunteer service, attendance at, or participation in the Event, to the fullest extent permitted by law. I hereby grant permission for my name, image, likeness, voice, and appearance to be photographed, filmed, videotaped, recorded, and otherwise captured in connection with the Event, and I authorize the Released Parties to use, reproduce, publish, display, distribute, and otherwise use such materials, in whole or in part, in any medium now known or later developed, for lawful promotional, educational, archival, publicity, fundraising, and event-related purposes without compensation or further approval. This permission includes use in official documents, news releases, advertising, promotional materials, websites, social media, television, radio, and other media related to the Event and the mission of Propel Forward, Inc. In the event of an illness, injury, or medical emergency arising during or related to my volunteer service or participation in the Event, I authorize the Event Director and/or Event personnel to obtain medical care for me from any licensed hospital, clinic, emergency responder, or physician deemed necessary for my immediate care. I understand and agree that I am solely responsible for payment of any and all medical services rendered to me, including but not limited to emergency transport, medications, treatment, and hospitalization. As it applies to my volunteer service, attendance, or participation in the Event, I agree to follow applicable public-health guidance and any health, safety, operational, traffic, access, security, or airport-related rules, directives, or protocols issued by Event management, the venue, the airport, or local, state, or federal authorities. I understand that the Event reserves the right to postpone, modify, interrupt, or cancel the Event, in whole or in part, due to circumstances beyond its control, including but not limited to weather, natural disaster, emergency, airport operational requirements, government order, public-safety concerns, force majeure, or any other condition that, in the judgment of Event management, affects the safety, feasibility, or operation of the Event. By signing below, I acknowledge that I have read this Volunteer Release and Waiver of Liability, Assumption of Risk, Medical Authorization, Media Release, and Volunteer Agreement; that I fully understand its terms; that I understand I am giving up substantial legal rights by signing it; and that I sign it freely and voluntarily without inducement.
